You will report to leadership with a clear path to HR leadership as the organization expands.

Location: Phoenix, AZ (onsite)

Compensation: USD 65,000 - 85,000 per year

Benefits

  • Ownership: You will lead both core functions with autonomy to shape them from the ground up.
  • Growth opportunities: Direct access to leadership and a defined trajectory toward HR leadership as the organization expands.
  • Professional development: Commitment to ongoing learning and support for relevant certifications such as NAMSS (CPCS/CPMSM) or SHRM when aligned with the role.
  • Impact: Your work directly speeds provider onboarding and patient access to care.

About the role

We are a growing healthcare provider group delivering care across partner post-acute facilities including SNFs, ALFs, and IRFs. We seek one capable, organized individual to own two connected functions: internal HR operations and external facility credentialing. This build-and-run role offers real ownership and a clear path to scale with the company. You will ensure provider support, compliance, and timely access to buildings and the EMR so clinicians can see patients. If you are early in your career but want broader scope and more autonomy than a large system typically offers, this position is designed for you.

Responsibilities

  • Serve as the primary contact with each partner facility's medical staff office, coordinating credentialing and privileging for all SNFs, ALFs, and IRFs we work with.
  • Prepare, submit, and track privileging packets on each facility's forms and timelines; follow up on outstanding items to keep onboarding fast.
  • Coordinate EMR access provisioning for newly credentialed providers and promptly deprovision when a provider leaves or a partnership ends.
  • Maintain a reliable, up-to-date tracking system showing which provider is credentialed at which facility, expirations, EMR access, and in-progress items.
  • Manage re-appointment and re-credentialing cycles proactively to prevent lapses.
  • Partner with the billing team to align payer enrollment and maintain data consistency across both functions.
  • Run day-to-day people operations, including onboarding, offboarding, recordkeeping, and HRIS administration.
  • Administer benefits and coordinate open enrollment.
  • Support recruiting and hiring across the practice.
  • Handle employee relations and serve as a trusted first point of contact for staff questions.
  • Maintain HR compliance with Arizona and federal employment law.
  • Contribute to building and refining people policies and processes as the organization scales.

Requirements

  • Hands-on experience with facility credentialing, privileging, or medical staff coordination across multiple sites is preferred.
  • Strong project-management instincts, comfortable juggling multiple deadlines and following up with external partners.
  • Excellent organization and a track record of maintaining accurate tracking systems.
  • Solid written and verbal communication, as you will represent the organization to partner facilities.
  • Working familiarity with core HR functions and a willingness to deepen HR expertise on the job.

Technologies

  • CAQH
  • HRIS platforms
  • Common facility EMRs
